Thank you for the excellent pics which reveal periobital alopecia and lichenification (thickening and darkening of the skin) characteristic of a blepharitis - - inflammation of the eyelids that may be due to a bacterial or yeast infection or may be secondary to an underlying condition such as a parasitic (demodicosis), allergic, or autoimmune skin disease, or leishmaniasis (rare in the United States). Here are the basics for treatment and prognosis:

1) Any underlying cause should be identified and addressed. Beau's vet needs to skin scrape looking for the Demodex mange mite and perform a cytology (microscopic exam of a small sample of skin surface) looking for abnormal numbers of bacteria and yeast.

2) Any topical medications should be discontinued if contact dermatitis is suspected.

3) If pruritic (itchy), an Elizabethan collar should be used to prevent self trauma.

4) Warm water compresses should be applied to affected areas 2-3 times daily to decrease swelling and remove exudate (discharge).

5) If bacterial infection is present, a topical antibiotic-glucocorticoid ophthalmic preparation should be applied to the affected eye every 8-12 hours for 2-3 weeks. Effective preparations include those containing the following: bacitracin-neomycin-polymyxin-hydrocortisone; neomycin-prednisone; gentamicin-betamethasone. Don't use if contact dermatitis is suspected.

6) For bacterial blepharitis, appropriate systemic antibiotics should be administered for at least 3 weeks. For yeast, ketoconazole dosed daily for 30 days should suffice.

7) For autoimmune blepharitis, treatment with immunosuppressive medications should be provided.

Please note that if the anti-itch pill is a glucocorticoid (steroid) such as prednisone or, instead, oclacitinib (Apoquel) either drug will predispose to secondary bacterial, yeast, or mite infection.
