Hello, my dog is 13 years old and his ALT level has been going up and down... 158U/L on 11/22, 200U/L on 3/10, 228 on 3/14 and 171 today, 3/19 is this normal? we are monitoring his ALT because he is on medication

Veterinarian's Assistant: I'll do all I can to help. What is wrong with your dog?

Besides elevated ALT, his blood test has been normal

Veterinarian's Assistant: Where does your dog seem to hurt?

Nowhere really... just front leg joint. Doctor says it arthritis and he's been getting injection once a month

Veterinarian's Assistant: OK. What is your dog's name and age?

His name is ***** ***** turning 13 this month

He has been on Clomipramine since June 2017

Clomipramine since last June, and he started getting Cartrophen injection in december 2017

I understand thank you. Carprofen can be very hard on the liver. You might ask your vet if you can switch to Meloxicam. It's much safer for the liver and it's cheap (ask for a prescription for the generic and go get it at your pharmacy, not at the vet's office). The liver can heal itself, after you stop the Carprofen the ALT may normalize on it's own. He should also be taking Milk Thistle and SamE for liver health.

If the ALT continues to go up though, then have the liver ultrasounded to be safe.

We did the US on 3/10 and Vet said there are some uneven colors... indicating there is a possibility of problems with the liver and spleen... but he said US was not clear, so I took Baileys in for a CT this morning. I insisted on liver test again and that's when i saw the ALT has gone down...

CT results will not be out for another couple days

Here is the dose for SAMe:

Daily dose for animals with body weights of:

  • ≤12pounds (5.5 kg): one 90 mg tablet;
  • 12-25 pounds (5.5-11 kg): two 90 mg tablets (or one 225 mg tablet, if more convenient);
  • 25-35 pounds (11-16 kg): one 225 mg tablet;
  • 35-65 pounds (16-29.5 kg): two 225 mg tablets;
  • 65-90 pounds (29.5-41 kg): three 225 mg tablets;
  • >90 pounds (41 kg+): four 225 mg tablet

Milk Thistle is normaly between 10-50 mg/kg daily

That sounds great. Once you get the CT results, you will no if there are any masses to be concerned about. If not, then I would recommend switching arthritis meds and adding the 2 liver protectants like we talked about.

Yes, this is very common with Carprofen. It is one of my least favorite drugs. But luckily there are good alternatives.
