I'm sorry to hear of this with your dog. You've described hematochezia - blood through the anus with or without feces. There are quite a few considerations when this is seen. They include anal sac gland impaction, sacculitis, or abscess often evidenced by "scooting" behavior, licking, chewing at hind end, and a foul odor; perianal fistula - German Shepherds are predisposed; neoplasia - more common in older dogs - perianal adenoma and anal sac adenocarcinoma are most common; anal stricture (uncommon); trauma - passing foreign material through the colon/rectum/anus; proctitis; colitis (important - idiopathic/unknown cause, food allergy, infectious/bacterial, parasitic - hookworms, whipworm, Giardia, stress-associated, inflammatory bowel disease (IBD).

Your vet will want to visually examine and digitally palpate (feel) your dog's rectum and anus. Presumptive therapy for those parasites may be prescribed. Additional diagnostics would be predicated upon findings in your dog's initial exam. Please respond with further questions or concerns if you wish.
