Chronic ear infections and...

Chronic ear infections and within a day of being off of medicine her ears get extremely inflamed... she is always in so much pain

Paisley, age 7 and is a cocker spaniel and I know they are prone to ear infections

Veterinarian's Assistant: Is there anything else the Veterinarian should be aware of about the dog?

She has been on a special diet for over a year as I have been told her ear infections are due to allergies She is on ZD

Have you had the ears cultured to see what bacteria you're dealing with?

Also, have you done oral antibiotics or just topical meds in the ear canal?

I have had her ears cultured a few different times over the last year. A lot of it is yeast but sometimes there is multiple types of bacteria. She is on topical and oral antibiotics. I have found that the best thing that helps sometimes is making my own solution with apple-cider vinegar, tea tree oil and hydrogen peroxide. She is on 250 mg of Cephalexin 2x a day and Otibiotic Ointment. She is also on Apaquel 2x a day

Yikes, ok she has been through everything. Your vet is treating pretty aggressively already.

Unfortunately, your next step is surgery. She sounds like the ideal candidate for a TECA (total ear canal ablation)- likely bilaterally. This is where all the ear canal tissue is surgically removed. I'm sure she has lost a good portion of her hearing already, so the transition shouldn't be too hard on her. She'll still have nerve hearing, but it'll be limited.

I don't see this ever clearing up completely with medical management alone. If I were to try medical, I would do a full 30 days on both Cephalexin + Ketoconazole (anti-fungal) and I wouldn't put anything into the ear canals.

It's a big surgery for sure, but it cures these dogs with chronic ear infections. It's not an easy decision to make at all.
