Summary: 12 year old lab-hound mix with episodic 24-hour+

Summary: 12 year old lab-hound mix with episodic 24-hour+ bouts of vomiting, gagging and drooling, present since rescued at age 3. No vet or specialist has been able to diagnose or treat.Details: Dog will be completely fine, then all of the sudden his GI tract will start to make audible churning sounds, he begins drooling, then licking and attempting to ingest almost anything he can get mouth on, followed by vomiting, severe gagging and drooling. The vomit is sometimes partially digested and eventually becomes just bile and saliva. He continues to gag and dry heave to varying degrees of severity. He does not lose his appetite. These episodes will endure anywhere from hours to days. He has gone for periods of almost a year without any episodes. He has currently been having episodes about every over day for over two weeks and it has never endured this long. I have never been able to correlate onset of symptoms with any activity or environmental factor. Onset appears 100% random.Unsuccessful treatments:He has previously had his stomach tacked, exploratory surgery, many x-rays and ultrasounds. He has tried various specialized diets including high motility varieties. He has taken probiotic supplements. He has taken phenobarbital to treat possible vomiting as partial seizure. He takes 1.5 twenty-five mg tabs of amitriptyline 2x per day. He has tried pepsid, pepto and prilosec. He has been given low does of diazepam (no impact of any kind).Very desperate to solve this chronic mystery disease.

Hello. This is Dr. Hill. He is a very challenging case. What did the biopsies of his intestinal tract show when he had abdominal surgery?

Hi Dr. Hill. Since that particular surgery was so long ago (7+ years ago) and I don't have the records with me at the moment I can't recall exactly what testing they did; however, no anomalies have ever been discovered during surgery, exams/xrays or testing. Blood tests have always been within expected ranges.I also very egregiously forgot to mention that he quickly becomes severely bloated/distended once the episode begins. He frequently burps up air (that I think he has gulped down while gagging and licking?). This endures throughout the entire episode.With the notable exception of this mystery illness and some resulting slight weightless, he is in extremely excellent shape for his age, with no joint or other age issues.

It sounds like an unusual manifestation of neurologic disease with the motility of the intestinal tract impacted or episodic inflammatory bowel disease flares. Did you ever try any steroids?

I believe he was given prednisone many years ago during one episode, without impact. However again I'm not sure the exact dosage and he has not taken any steroids for any length of time.

What a handsome boy. It sounds like he is having motility issues. You did not mention any promotility drugs. At a minimum, you should try reglan for a couple of weeks. Also, Cerenia would help with the acute vomiting. The two could be given together.

Oh my gosh, I did not mention any but he has in fact taken both Reglan and Cerenia. We gave him shots. There was no effect. Sorry for the complicated history.

His diet also seems to have no impact. We tried a variety including high motility.

With the diet, did you do a full 8 weeks. Also, did you do a prescription hypoallergenic diet, like science diet ZD or Ultamino?

he did more than 8 weeks for all the foods tried. He did have prescription food including low residue and some other varieties that I included hypoallergenic.
