Customer Question
This condition called "reverse...

This condition called "reverse sneeze" sounds more like a severe asthma attack in my older dog. This scares and it sounds like his nose is stuffed up but, you can't get a dog to blow his nose so what can I do to lessen this happening?

Veterinarian's Assistant: I'm sorry to hear that. What is the dog's name and age?

His name is ***** ***** I don't know his true age as he and his sister were adopted from a kill shelter. Our local vet seems to think he may be between 8-10. He is about 20 pounds. He is a short legged terrior with blond hair and a pink nose (someone suggested Chihuahua in his line) but because this gasping for air happens all day and into the night when we are sleeping, I can't help thinking it is more than just a reverse sneeze. I live in high desert and we have sage pollen and tree pollen counts along with ragweed issues currently. He is NOT an outdoors dog and spends 20+ hours sleeping.

Veterinarian's Assistant: Could be a lot of things that cause lethargy. The Expert will know how to help Benny. Is there anything else the Veterinarian should be aware of about Benny?

He had blood work done last week to determine why he is so tired all the time and they had several thoughts that will be determined with another blood panel in three weeks time. Nothing conclusive came of this test.

Greetings, I’m Dr. Peck, a small animal veterinarian in general practice. Hopefully I might be of some help. One moment while I reply…

Disclaimer: This communication is for discussion purposes only. Without seeing an animal in person, a valid veterinary-client-patient relationship does not exist, and therefore I am unable to diagnose, prognose, or treat. Any comments made during discussion here are based on the information provided and are simply my thoughts regarding what I’d be considering were this type of case presented to me in person. The BEST recommendation for ANY veterinary-related concern is to seek veterinary assessment in person with your local veterinarian.

First thing that should be tried is Benadryl (Diphenhydramine) 25mg tablets. For a 20lb dog, if these are severe as reported, I'd start at 2 tablets by mouth every 12 hours. If improvement / resolution occurs, then decrease to 1 tablet by mouth twice daily. If resolution persists at this dose after a few days, you might try once daily. All of these doses are safe and could be done long term. Sedation / sleepiness is a common side effect that often lessens with time (days / weeks on it).

I have seen difficult "reverse sneeze" cases that ended up being related to other pathology - such as laryngeal paralysis, chronic bronchitis, or, most noteably as this case really seemed like a simply reverse sneeze until the dogs nose was scoped - nasal mites. Trial treatment for nasal mites might be done with revolution or ivermectin - ask you veterinarian about this if this hasn't been considered. Other diagnostics that could be pursued could be chest x-rays, nasal / head x-rays, nasal scoping, advanced imaging. These are typically not necessary to pursue however except for rare cases. If discharge is present, especially bloody, scoping / biopsy should be considered along with fungal cultures.
