My name is***** and I am sorry to hear that Jack is so sick. Based on your description the following possibilities come to mind:
-back or neck pain. This is very common in older small dogs and if it is a disc pressing on his spinal cord, this can affect how he can walk and move. If he is very painful from this, he may be "zoning out" to deal with the pain.
-liver or kidney disease
-pancreatitis causing severe abdominal pain
-cancer causing weakness
I would recommend taking him to your vet for an exam and consider blood work and x-rays based on the exam findings.
