Have skin scrapings, to look for microscopic parasites been done?
There is a new medication called Apoquel that is helping a lot of itchy dogs. You have to get it from your veterinarian.
Some dogs are itchy because they are allergic to an ingredient in the diet. Trying a "limited ingredient" or a hydrolized diet might be worth trying.
Are you using a flea and tick product? If she is hypersensitive to flea bites, even one or two bites could set her off itching all over.

I thought of a couple of other things: If Benadryl does not help, you could try 5 to 10 mg of Claritin once a day.
Seeing a veterinary dermatologist may be helpful. Where in Utah are you; I could see if there is one near you.
