I have a sharpie mix who is on zonisime for seizure control. She is 40 lbs, 11 years old, otherwise great lab panels. She is having some breakthrough seizures, 2 this month. She is taking 100 mg, q12 hrs. Can I increase to q8 hours?

I'm sorry that Ming is not apparently well controlled with her anti-seizure medication.

If this were my case, I wouldn't increase the dosage amount to three times a day (for a total of 300 mg/day). Rather, I would consider increasing the amount she's receiving every twelve hours to 150 mg instead. This dosage is consistent with what is recommended by Plumb's Handbook of Veterinary Medicine.

However, having said the above, I would probably run Zonisamide blood levels before increasing the drug to determine if my patient was in a therapeutic range on the twice a day dose.

I do know that not all vets will run blood levels, though, before increasing this particular medication.
