My dog, a six-year-old male cocker spaniel, seems to have something

My dog, a six-year-old male cocker spaniel, seems to have something lodged in his throat.
His behavior is normal in every other way, (i.e., eating, barking, wanting to play, etc.}, but then he starts, all of a sudden, heaving his diaphragm in and out intensely, culminating with an action akin to vomiting. But he doesn't actually bring anything up, just some milky liquid sometimes, sometimes just "dry heaving".
I took him to a vet last week, but the meds haven't made any difference.
I recoil at any kind of surgery, but what's going on? What should I do?
