Four year old shih tzu feels like he has fever is scratching until raw and I can't reach his vet. He is still eating but terribly lethargic . Please help

Welcome! My name is***** am a UC Davis graduate, and currently, a Medical Director of a veterinary hospital.
I am sorry to hear about this concern for Sampson. Benadryl was a wise choice. Not sure how much you gave but the goal is 1mg per pound of body weight.
So, if about 8-12 pounds, give at least a 1/2 tablet of benadryl 25mg. If anywhere near 25 pounds, just give a full tablet.
But, fever?
Try to get the exact temperature. Any digital thermometer will work a take a rectal temperature. If over 103.0, you really need to get in to any vet today for supportive care.
If he is having an allergic reaction, he will desperately need a steroid shot and/or some oral steroids to quickly get him comfortable. The benadryl, by itself, won't be enough.
A cool oatmeal-based shampoo bath may be of some benefit as well. Always make sure to wash all shampoo off thoroughly.
