Its a case study for my class and I am having a hard time finding an answer, here is what it says below.

A fouteen year old miniature poodle, presented to the emergency service this morning after ingesting some rat poison 3 to four hours ago. The owner induced vomiting with hydrogen peroxide and feels that the dog vomited up 90 percent rat poison. As of 30 minutes ago the dog was still vomiting. Every time the dog was offered water, it would drink and promptly vomit, the last vomiting episode occured at approx 20 minutes ago and still contained chunks of rat poison.. The dog has a history of seizures and also diagnosed with the heart murmur. She was currently on phenobarbital 15 milligrams Q12H PO. Furosemide 12.5 milligrams 1/2 tablet Q12H PO. And Dnalapril 1.25 MG Q24H PO. When physical exam she was BAR,T=107, HR=110 R=40 MM=Pink, CRT less then 2 seconds, weighs 10 lbs, dehydration was estimated at 5-7 percent. Eyes were sunken: Lenticular Sclerosis was present OU. She is experiencing ataxia, especially in hind quarters.. The remainder of the physical exam was the normal limit. Are there any special precautions that need to be considered when treating her for her rat poison? Please help.....lolol

Ok, thanks.

The first thing we need to know is if the rat poison was the neuro-toxin or the anti-coagulant type. I'm going to assume anti-coagulant as that's the most common. With early detection, we can treat by inducing vomiting, giving activated charcoal and then starting oral vitamin K for 3-4 weeks.

With a seizure dog, I would just continue the Phenobarb but make sure it's ~ 4 hours before or after the activated charcoal is given to make sure it gets absorbed. That's the only thing different with the seizures.

With the heart disease, we need to be a little careful with the charcoal. Activated charcoal can cause hypernatremia (elevated Sodium). I would go toward the low end of the activated charcoal dose and only give it once. I'd also give one with Sorbitol so that it moves through a little faster.

I hope that is a typo and the temp is not really 107. If it is, then this is a critical dog that needs aggressive fluids, plasma, ect.

With this amount of vomiting, I would give an anti-emetic (Cerenia or Anzemet) prior to giving activated charcoal. I may also give some sub Q fluids to help with the estimated dehydration or just let the dog eat/ drink normally after the charcoal starts moving. Low rate IV fluids overnight may also be an option but we hardly ever hospitalize dogs for acute ingestion of rodenticide- usually they are treated out-patient.
