I have a 2004 dodge 3500 I have had it to the dealer a number of times never fixing anything

Mechanic's Assistant: What is the complete model and serial number of your machine?

Here is the problem while driving about 60 mph the truck will shift down and slow to about 45mph before the throttle will pick up, and at slow speeds the truck bucks throttle up throttle down sometime not working at all for it seems like minutes

There are quite a few things that could be going on and it's going to require some testing to determine exactly what is wrong. You may have a fuel delivery issue, it could be an electrical problem. There are several sensor signal issues that could cause this.
