Truck threw a 2262 code. Expected it after 140,000 miles.

Truck threw a 2262 code. Expected it after 140,000 miles. Changed out the turbo and cleared the code. Ran the truck and it immediately threw the 2262 code again and went back into limp mode. What else is in this system that could cause this. Having trouble believing I installed a defective turbo from Holset.
JA: Just to clarify, what's the exact code? And have you tried to reset it?
Customer: P2262 and yes I cleared it twice.
JA: What year is your Turbo?
Customer: Not sure, bought it from O'reilly auto parts
JA: Are you fixing your Turbo yourself? What have you tried so far?
Customer: Complete turbo replacement, cleaned all oil and cooling lines, replaced oil and filter and new coolant. Everything looked good going in.
JA: Anything else you want the mechanic to know before I connect you?

Hello, my name is ***** ***** I will assist you today. A P2262 is almost always a bad turbocharger, or a sooted up turbo. Never ever see it being anything else. The diag procedure has you first clean it. If the code returns, then you replace it. I think you got a defective one. Get one from the Ram dealer. They are rebuilt/remanufactured by Cummins and have never given me any problems.

Cleared the codes; separated the exhaust system before the dpf (clogged); started the truck. Didn't pop a code except for the clogged dpf but still surges and will not exceed 1500 rpm.
