Hello and welcome to JustAnswer!
If it's falling out of gear then an internal issue is likely but it's more likely that the trans is up and downshifting on it's own. This could be caused by quite a few things but is most often caused by an erratic accelerator pedal position sensor signal.
Is it up and downshifting on it's own while driving at a steady speed?
