I suspect I have fingertip eczema. The symptoms are dry, thick cracking fingertips. It seems to get worse when wet but better with 3x applications of heavyduty Neutrogena (like vaseline). Would an over the counter eczema solution be a reasonable first step to treating this?

Doctor's Assistant: Can you tell me about all the symptoms you've noticed (e.g. itching, rash, new birthmarks or moles)?

No itching really, more like a slight burning. No new birthmarks or moles.... I did have a onetime only contact with a street cat which had a parasite and lost its fur. (This was more than a year ago though.)

Doctor's Assistant: How long have you been dealing with the burning? Are you experiencing any other discomfort?

Recently. About 1-2 weeks.

Doctor's Assistant: Anything else in your medical history you think the doctor should know?

I went through menopause last year.

Welcome to JA and thanks for this question. Im sorry to hear about your symptoms. Can you attach a picture of the fingertips here which will help in determining if this is likely to be eczema or not?

Since you seem to be offline, ill leave a response here which we can discuss later on.

Usually fingertip dermatitis or eczema would cause the following symptoms,

  • Dryness
  • Scaling of the skin
  • Cracks or fissures in the skin
  • If redness or pain develops in the fingertips or if they become swollen then that could be an indication of a secondary bacterial infection.

Now the treatment of this condition would involve a number of methods. This would require you do the following,

  • Minimise contact with irritants — even water
  • Use non-soap cleanser when washing hands, rinse carefully, and ensure hands are completely dry afterwards
  • Completely avoid touching allergens that have been identified by patch testing
  • Wear task-appropriate protective gloves
  • Apply thick emollients before work and reapply after washing or when the skin dries out (this may be 10–20 times in a day).

But you are right, it IS a reasonable first step to start using OTC treatments. You should start using OTC hydrocortisone along with the above measures and see if that helps. Secondly, i would also recommend that other causes for the dry skin be considered including the menopause itself as well as a other hormonal conditions like thyroid disorders, hypothyroidism can certainly cause this problem in many patients. It would be worthwhile to consider those as additional possibilities as well.
