Can you post pictures of his rash ?
The locations sound suspicious for eczema as common ( elbows and knees ) that if his cause is combination of dryness and allegic type sensitivity ...if this a combination of moisturizing lotion plus a mild steroid cream twice a day ( OTC 1 percent hydrocortisone ) may help

Does rash itch ?
You may be able to attach a picture file if your toolbar above where you type has paperclip icon ..the tap that and It may ask what file to attach and go from there ..
You may send to address on site such as photobucket.com and give me address ( may take me longer to get to that however....)

So far I suspect stubborn case of eczema but want to get a look ...email does not come up to the page I am responding on here ...
His doctor may prescribe stronger steroid as the OTC 1 percent hydrocortisone is weaker than Rx versions ...( Kenalog , Elocon many others )
Trial of Sarna lotion or Aquaphor / gel with aloe may help too
Twice a day moisturizer applications ....
