Hello,You would simply contact the prosecuting attorney's office in the jurisdiction where the alleged crime was committed, and discuss the matter with him/her. The prosecutor has the statutory duty/responsibility of presenting cases to the grand jury, so he/she would be the person to contact. A certain procedure for doing so is not necessary.Hope this helps.
