If so, then the PO has a choice whether to press someone in your situation to go back, or, to initiate the procedure to revoke parole.
If so, then it would be either up to an administrative board (akin to the one that granted the parole in the first place) or a Judge to decide on what to do, and they may:
1) Extend the parole and give you additional time to complete the SAP;
2) Revoke parole; or
3) Add on additional requirements to keep the parole going, and/or, extend it as needed.
So really, it is at their discretion.

Yes. If one is on more or less good terms with the PO, it may be worth asking the PO not to file to revoke, but to work with the individual to allow them to make up any issues with the requirements of the parole.
The PO may give one a second chance.

Even if you have served out your time, if SAP was part of your parole, and it was not concluded, it may revoke/add to your parole, or incarceration, I am afraid. That is, if the PO refuses to work something out and begins the revocation process.
