what is the leagal ramifications for falsying a signature

Resolved Question:

I work for an occupational health nurse who has her own business. I believe she copied a patients name from another form and copy it a form with my signature. She than faxed it to the patients phyisician to obtain information for a workman comp claim. Can I be held liable?

No because you didn't perform the act. As a practical matter, wasn't she supposed to get the MD's records for the comp claim of the patient? Why did she do what she did?
