There are no laws prohibiting an employer from keeping notes about employees - however, what is the reason the employer is doing this - is there a dress code that needs to be adhered to? Or is there a sense that this is some sort of sexual harassment?

Well, they are within their rights to make notes for whatever reason - other than something that would infer a bona fide discrimination factor such as sex, age, race, religion, disability.
