This could be a few different things.
Could be a bad disc.
Could be a dirty Laser Lens (need to be blown out, or cleaned)
Could a software glitch. (maybe a Reboot of the machine will clear it up)
YOu could try to change your performance settings, so that your machine devotes more CPU and RAM to doing tasks, instead of focusing on the asthetics. Like rounded off windows.
Its really hard to know EXACTLY what the issue is. As you stated, turning off HW acceleration, helped....but was not a fix.
Try adjusting your performance, and see if that helps.
From there...Blow out (with compressed Air), and make 100% sure the DVD/Blu-Ray burner is clean!
