In order to hack your phone, it must be jailbroken - a process that circumvents Apple's security protocols and allows installation of unsigned applications.

Jailbreaking leaves a very clear sign behidn - an app called "Cydia".
Check your apps, and see if you can find Cydia, which has this icon:

If there is no Cydia, your phone was NOT hacked.
