Dx: inflammatory bowel disease with chronic diarrhea, vomiting
Tx:
prednisolone 5 mg, 1 tab SID
Cerenia 16 mg, 1/4 tab EOD
vitamin B-12, 0.25 mL subQ once a week
(have been erratic for several months; my compliance improved last 3-4 weeks)
Probios, 2 g once a week; right now SID because of antibiotics
Clavamox, 1 tab BID, finished 7-day course on FridayProbably three weeks ago, Archie stopped washing his ruff. I wasn't sure why, but he was eating well and wasn't dehydrated. Two weeks ago (Thurs 10/28), he started snorting, and sneezed out some yellow green snot. His eyes were also crusty, and he's got chin acne since he wasn't washing. At that point, he wasn't eating well, either. Since he's on on pred, I took him to the vet -- I was lucky they were able to squeeze him in.The vet (not our usual one) prescribed Clavamox, with Probios (we keep on hand) several hours after to help his intestinal flora. She suggested witch hazel for his chin. (Haven't gotten that yet.) We discussed his eyes, (I keep Terramycin on hand) but he wasn't having green crud, so I opted for keeping an eye on him. She also spent a good deal of time talking about putting him on a hydrolyzed protein diet to see if we can wean him off of the pred. (It would be expensive; but so are the meds ...)Archie gradually improved -- by this past Tuesday, he'd pretty much stopped snorting. His appetite has improved, and he's washing again. He did have diarrhea (need to shave his fuzzy butt), but he always has some.I figured that he was convalescing well, but this afternoon when I went to do meds, his left eye was weeping khaki-green gunk, and his conjunctiva are red quite swollen. I started him on Terramycin, both eyes.Questions:
How many times a day?
And how much (length of ointment)?
I gave about 3/8 inch in the unaffected eye, and 3/4 inch in the left eye.Could this a recrudescence of FHV? Or is it likely bacterial?
Since we got Willie -- who had a severe FHV infection as a kitten and has symblepharon, I've noticed more and more of the cats having crusty eye boogers on a regular basis, which makes me think that Willie might be shedding the virus and giving it to everyone else. (That also might be one explanation for why we're going broke pulling teeth with cats with stomatitis ...)So, that's Archie. Yesterday afternoon, Squeak started snorting and sneezing. I believe that the prednisolone brought feline herpesvirus (FHV-1) out of its carrier state and his conjunctivitis is also from that virus. I like the use of Terramycin ophthalmic ointment because FHV-1 conjunctivitis can be complicated by secondary bacteria. Dosing is 1/4 " on to the globe of the eye thrice daily for a minimum of 10 consecutive days. You can bet that every one of your cats is a carrier of FHV-1.

When reasonable - and I don't know how you'll go about doing it - a hypoallergenic food trial is always smart when dealing with IBD.

Well, I'm reminded why it takes half or three-quarters of an inch of ointment to get 1/4 inch in the eye. Archie was fine for the right eye, but the first try applying the Terramycin to his left eye -- which is clearly painful -- ended up in his eyebrow. Sigh.I'm thinking we should try a hydrolyzed diet, too. But I'm not ready to deal with any new project before my tests on Tuesday. May I bring it up with you again?Thanks again for the dosage for the Terramycin. I'll post positive feedback.Cathy

Archie's conjunctiva are less red, though still swollen. There are little lumps on the swollen tissue. The left eye is weeping less, but still very sore. This afternoon it took two tries to get the goop in his left eye.C.
