Male older cat. Got a cat bite on mid left side. About 5 days ago. Now some tenderness and swelling about the size of a silver dollar. He is eating OK still purring

Veterinarian's Assistant: I'll do all I can to help. Strange behavior is often perplexing. I'm sure the Veterinarian can help you. What is the cat's name?

Do I need antibiotics? I put some cream on him.

Veterinarian's Assistant: Is there anything else important you think the Veterinarian should know about the cat?

I'll follow up for DocPaul for you. To answer you directly, yes, he needs a systemic antibiotic at this time. In general, such an antibiotic needs to be given within 48 hours of a bite lest cellulitis (inflammation of the tissue) progresses into an abscess. The pus inside an abscess doesn't allow an antibiotic to penetrate into the area and so an abscess needs to be lanced, flushed with an antimicrobial solution such as chlorhexidine, debrided (devitalized tissue removed), and the wound is either left open to drain or a drain is sewn into the wound. Topical treatment such as with creams or ointments can't penetrate under the skin where infection is festering. Your vet will assess whether or not opening up this area is needed or, instead, a systemic antibiotic is still likely to resolve the infection. You can purchase injectable penicillin in feed stores (or online). It's not the best choice for these infections but may suffice. Dosing is 10,000 units of procaine G penicillin per pound of body weight once daily subcutaneously for 5-7 days. A 3 cc 22 ga needle syringe with extra needles can be purchased as well.
