My 13-year-old Maine Coon mix has had a respiratory infection for about a week. He's been on azithromycin for about four days, but his nose is still oozing snot and he's still congested. Should I ask my vet to consider a different treatment?

Hello & welcome, I am Dr. B, a licensed veterinarian and I would like to help you today.

What tests did Cato's vet do?

What color is the nasal discharge?

Are his gums pink or pale/white?

Can you take a breathing rate for me (just count his breaths for 10 seconds + multiply by 6)?

Tests: Standard blood work--everything was normal. The emergency vet also did a chest x-ray, which was also normal. Heart and lungs sound fine.Nasal discharge is light greenishVet said his gums may be slightly blue, but it's hard to tell since Cato is gray and his gums normally have a bluish tint because of his colorationBreathing rate is about 30 breaths per second, but breathing seems labored. My vet said when he listens to Cato's nose, he hears more noise when he's exhaling than when he's inhaling.

While culturing the nasal cavity is generally not helpful it can be considered and if a pure bacterial culture (uncommon) is found, a bacterial culture and sensitivity will tell us which antibiotic would be best to prescribe. Geriatric feline herpesvirus (FHV-1) should be an important consideration. Testing for FHV-1 is problematic in the adult cat because of previous vaccinations against FHV-1 and so many of us will presumptively prescribe the antiviral drug famciclovir to these cats. It can be formulated into a tasty suspension by a veterinary formulating pharmacy which should be done because you'll need to dose thrice daily for 2-4 weeks. Please respond with further questions or concerns if you wish.
