He has always been...

He has always been a picky eater and has always had breathing issues (long story, has to do with the fact he was feral) but recently he has been vomiting bright yellow, foul smelling, not a lot, just a bit. Just curious if this could be normal

Sabot is 6 years old

Chronic nasal issues stemming from when he was a feral kitten. He had phenomia (sp) and almost didn't live as a kitten (we found him 30 miles in country, all other kittens dead from freezing, mother ran over on road). So, he does have breathing issues.

I've just always been worried about him, as he's not exactly an active cat. I have a 20 year old cat that is as active as Sabot. I've had cats all my life and I know they sleep a lot, but Sabot has always slept more than any other cat. And sometimes his head is hotter than normal. The window in which he sleeps, is often sprayed with nasal stuff/snot. I worry that his nasal issues is causing him issues with his tummy.

I'm sorry that your question wasn't answered in a timely manner. We don't expect respiratory disorders to affect the gastrointestinal tract but there are systemic infections such as FeLV (feline leukemia virus) and FIV (feline immunodeficiency virus) that can affect more than one body system. If vomiting has persisted for more than a few weeks, inflammatory bowel disease (IBD) is a very important consideration. This is an idiopathic (unknown cause) inflammation of the gastrointestinal tract but thought to be immune mediated. It usually responds nicely to a glucocorticoid (steroid) such as prednisolone and/or the antiinflammatory antibiotic metronidazole. The diagnosis of IBD is done by excluding other causes of such vomiting and that's done by performing a diagnostic panel of blood and urine tests and ultrasounding the gastrointestinal tract. Confirmation of IBD requires scoping and biopsy of the GI tract but if I see a good response to prednisolone or metronidazole, I'll make the call for IBD.

Differentials include a food intolerance and chronic pancreatitis or hepatitis - all of which can be ruled out by testing. Food intolerance requires hypoallergenic diet trials to rule in or out.

Food intolerance/allergy is addressed with prescription hypoallergenic diets. These special foods contain just one novel (rabbit, duck, e.g.) animal protein or proteins that have been chemically altered (hydrolyzed) to the point that Sabot's immune system doesn't "see" anything to be allergic to. The over the counter hypoallergenic foods too often contain proteins not listed on the label - soy is a common one - and these proteins would confound our evaluation of the efficacy of the hypoallergenic diet. The prescription foods are available from her vet. There are many novel protein foods and a prototypical hydrolyzed protein food is Hill’s Prescription Diet z/d ultra (a hydrolyzed protein diet is my preference because it avoids the possibility of my patient being intolerant to even a novel protein). A positive response is usually seen within a few weeks if we’ve eliminated the offending food allergen. Food intolerance can arise at any age and even after my patient has been eating the same food for quite some time.
