Customer Question
My cat is 15...

My cat is 15. He has a severe worsening limp of his front right leg for a couple weeks and now has been shown to have elevated CK and AST levels as well as lowered potassium. He has been on a step down regimen of prednisone which I thought was helping but now he seems to be worse.

Veterinarian's Assistant: I'm sorry to hear that. Did your cat have a fall?

Not that we are aware of but we do have steps and he likes to lay on the landing which is 6 steps high. He doesn't get on the bed or couch anymore due to his chronic arthritis which he takes glucosamine for. He is actually taking glycoflex now which has gluc, MSM, DMG, Perna and antioxidants for max arthritis support

Veterinarian's Assistant: What is the cat's name?

Smokey

Veterinarian's Assistant: Is there anything else important you think the Veterinarian should know about your cat?

He had a partially detached retina from high blood pressure so he now takes 2.5mg amlodapine daily which has stopped the bleeding behind his retina but he does have permanent vision loss. Not 100%...he can see but it is very impaired. So yes it may be possible for a misstep but we have never seen him take a wrong step causing injury. In the meantime we are keeping him away from the steps just in case

I'm sorry that your question wasn't answered in a timely manner. Please let me know if you still need help. May I have an update on Smokey, please?

Elevations in creatine kinase less than 1000 IU are usually associated with the restraint associated with blood acquision. Significant elevations suggests the presence of disease that results in muscle disruption. I would be concerned that neoplasia at Smokey's age and history of progressive lameness would also cause muscle disruption.

The AST isn't helpful in cats. It's not as specific for the liver as ALT which should be imore important to monitor. A decreased serum potassium is consistent with chronic renal insufficiency in a 15 year old.

My next diagnostic of choice would be to X-ray his right front leg. Please respond with further questions or concerns if you wish.

Is there a muscle disease that would cause abnormal gait on just one leg? I forgot to mention that he walks on his front feet and "wrists" rather than just on his feet. His regular vet said that his muscles and tendons appear to have a laxity about them. I read that muscular dystrophy is a possibility with elevated CK & AST. Is there a way to diagnose that in cats? Also would labs have indicated if there was a mass of Any kind or only if it is malignant?
Cat Veterinarian: Dr. Michael Salkin, Veterinarian replied 10 months ago

Yes, if a peripheral neuropathy were present - a disorder of nerve(s) innervating the limb. Systemic neuromuscular disorders in cats are rare and diagnosis of them can be challenging. Myasthenia gravis and muscular dystrophies affect more than one muscle/limb. Muscle biopsy is necessary to diagnose muscular dystrophies and the acetylcholine receptor antibody test is necessary to diagnose myasthenia gravis. You've described carpal joint laxity which is more often the result of trauma or geriatric breakdown in his ligaments. If labs involved a major organ, I would see evidence of damage to those organs but labs aren't going to helpful if the mass involves his cervical spine or limb; if a mass were in either location, paresis (weakness) or paralysis can arise. Please continue our conversation if you wish.
