My cat has always been good about using the litter box but lately he poops and pees on the couch, floor or our bed. What could be causing this?

Inappropriate urination and defecation is a frustrating problem. There could be a medical reason that Paco has started doing this, so a thorough check up with his vet would be a good starting point. Your vet can do blood, urine, and fecal testing to ensure that this new development is not due to an underlying medical issue such as an infection or organ disease. Your vet will also be able to rule out pain or arthritis causing Paco to not be able to make it to the litterbox. Alternatively, the reason Paco is doing this could be behavioral....stress, a new diet, a new type of litter or litterbox, a new pet in the home, or a change in his environment could trigger this new behavior.
