There seems to be a number missing in one of the trouble codes, P041, there should be a P and then 4 numbers.

The P01125 is a accelerator pedal position sensor code but I don't have any information about the P1041 code. Let me open it back up as there may be another expert who has more onit. The information I have is that the o2 sensor signals have been swapped
p041 is that the number two signals on each bank have been swapped so the o2 information for bank 1 is getting swapped with the 02 information with bank 2.
Have you had any work done on the exhaust?
That could be the cause if they were plugged in visa versa.
