But she cannot enter as a visitor with the intention of remaining. If she cannot show strong ties to back home they are not going to admit her at the border coming home with her new spouse.

So she better be prepared to show a job, a family, ties to the community, assets etc.

If she ends up being admitted and you start the sponsorship application she can remain. She cannot get medical coverage as a visitor or someone who will be in Canada without legal status. That is not possible.

She does not need a study permit if the course is under 6 months.

I doubt she will get the study permit to be honest. I hear from students each day now whose positions are much stronger than your spouse's who are not being granted their study permits.

To get a study permit she first has to show a letter of enrolment from a designated learning institution.

You can see if she will gain admission to Canada after you are married but if not then you should start the spousal sponsorship while your spouse remains in Japan right away.
