He has to list all his family members and has to list the children.
If the mother won't let the children have medical exams he can tell this to Canada Immigration and indicate that she has custody.
He doesn't have to say he will not sponsor them. He cannot do that of course as then he is bound by that as Canada Immigration shows no flexibility about this issue.
But just missing a medical exam will not necessarily doom his application because if he wanted to sponsor them later on they still need the exam then.

You can contact the Law Society and use their Lawyer Referral Service. You will be given the name of a lawyer and can consult with the lawyer and the first half hour will be free.

We cannot give out names but you can call several lawyers and find out what they will charge. It will likely be a flat fee and you don't need the most senior or experienced lawyer to do this. In fact a law more recently called to the bar who only does immigration law would be very good and not charge nearly as much.
