This is between XXXX (here after known as Rescue) and ________________________________________________ (here after known as Foster Home).
RESPONISIBILTIES OF THE FOSTER-TO-ADOPT HOME FOR ANIMALS
____1) The Foster Home must sign and return the Foster-To-Adopt Agreement.
____2) The foster home is responsible for transporting the animal to all necessary vet appointments. The Foster Home must follow the rescue’s veterinary care requirements. In case of a medical emergency, the rescue must be contacted immediately.
_____3) The foster home will provide a safe and loving home environment and will socialize said animal with humans and other dogs.
Puppies are not to be excessively crated (not more than 6 hours in a 24 hour period)
_____4) It is the foster home’s responsibility to evaluate the said animal’s activity level, temperament and personality traits to ensure their home is the best home for the dog.
_____5) The foster home will provide adequate food, fresh water, exercise, a healthy living environment for said animal.
_____6) The foster home will provide basic training or reinforcement for proper house manners or obedience skills, and house toilet training of said animal. This training and reinforcement will be done only by positive reinforcement. At no time will the foster home use punishment as a training tool. The foster home
must understand that teaching/training puppies to go outside/potty is a slow, consistent procedure and that accidents can happen every day for quite some time. In the case of adult dogs, accidents are common even for housetrained animals when moving house and adapting to a new routine.
_____7) The foster home will ensure that the said animal wears the Rescue’s ID tag and microchip (if chipped) at all times.
XXXX RESPONSIBILTIES TO THE FOSTER-TO-ADOPT HOME
_____1) The rescue will provide a complete copy of all medical records and evaluations to the foster home if available at the time when said animal is placed on their home and when the adoption is finalized.
_____2) The rescue will pay for routine medical care as it is needed. Any other vet expense beyond routine care must be pre-approved by the rescue. Any unapproved vet expenses, other than a bona fide emergency, will not be covered by the rescue and will be the responsibility of the foster home.
_____3) The rescue will maintain contact and provide counseling and advice to the foster home.
_____4) The rescue has the right and authority to schedule a visit or to have an unannounced visit at the foster home.
XXXX FOSTER-TO-ADOPT AGREEMENT
The Foster-To-Adopt Home understands that until that time the above mentioned dog, ______________________ will remain the property of XXXX as a dog in our foster program until the time that adoption takes place.
The Foster-To-Adopt home understand that foster dogs must be immediately surrendered upon request from an authorized XXXX Representative. If it is necessary for XXXX to take legal action to recover a Foster-To-Adopt dog(s) or otherwise enforce provision in this agreement, the Foster-To-Adopt home will pay all court costs and legal fees incurred.
The Foster-To-Adopt home will provide our Foster-To-Adopt dog with adequate food, fresh water, exercise, socialization, healthy living environment and positive reinforcement. The Foster-To-Adopt home understands that if these are not provided, XXXX has the right to immediately remove the Foster-To-Adopt dog.
The Foster-To-Adopt dog(s) will not be left alone outside, chained outside, or an outside tie-out at any time, nor will they wear a shock collars or any comparable devices. Furthermore, they will not be transported in the back of an open truck or similar vehicle.
At no time will a Foster-To-Adopt dog(s) be allowed to run free. A Foster-To-Adopt dogs(s) will be allowed off lead only in a fenced yard or secured area and while under close supervision.
A Foster-To-Adopt dogs(s) will never be allowed to remain outside throughout the night, or kept outside when the Foster-To-Adopt parent is not home. It is understood that all Foster-To-Adopt dogs will live inside the Foster home and not with a family member, friend, neighbor, or any other person who is not an authorized XXXX foster care provider.
If the Foster-To-Adopt dog(s) is stolen or lost the Foster-To-Adopt parent will immediately notify XXXX.
Foster-To-Adopt dogs(s) must never be given to any other party for any reason or to be transferred to another as they are the property of XXXX
The Foster-To-Adopt home will observe and evaluate the temperament, personality and activity level of each Foster-To-Adopt dog(s) for a period of time determined by the rescue________to determine of the Foster-To-Adopt dog(s) is the right dog(s) for my home.
Each and every clause, in entirety, has been read and understood by me/us. By signing this document, I/we agree to adhere to the terms and conditions. I/we affirm that I/we have never been charge with animal cruelty.
I understand that any foster animal in my care may be removed from my home at any time with or without reason by or on the authority of XXXX. I understand that XXXX Foster-To-Adopt animals do not belong to me or my family, and, therefore, all final decisions regarding the welfare of the animal will be the responsibility of this rescue.
I am aware there is no guarantee, warranty or full knowledge of any rescued pet’s health and temperament. I volunteer to accept the rescued animal in my charge as a humanitarian act and agree to release and hold harmless XXXX, from any and all liability or responsibility in connection with any pet(s) I agree to foster. Furthermore, I agree that volunteers/foster families accept liability for personal injuries or damages caused by a rescue dog. Volunteers agree by signing below that working with animals is unpredictable and the utmost caution must be taken in caring for foster animals
Additionally, I hereby certify that I am in good standing with the local rescue groups and veterinarians. I agree to uphold a professional relationship with every contact I have through the rescue, including, but not limited to vet clinics, distributors, potential adopters, other volunteers of this, and other rescues. Furthermore, I promise that I will not conduct myself in any manner which could reflect badly on the rescue or any of XXXX volunteers. Last, I certify that I am at least 19 years of age and have not been tried or convicted for an animal related offence.
