I am doing an investigation into conflict between an
I am doing an investigation into conflict between an employee and her supervisor. One of the issues causing conflict was that the employee had a pattern of abusing sick leave (ie. sick on Mondays and Fridays + always using her allocated 15 days, usually early in the year). The employee was asked to submit sick notes to cover a period of three days for which she was sick. The doctor note she submitted had the wrong dates on it, as they were for the week the after the employee off work. The supervisor asked the employee to get another note for the correct dates. This is only one of the issue of conflict that I am investigating, but when I look at both submitted sick notes, they look fraudulent to me. The black and white signature is not original and it is exactly the same on both sick notes. Both notes state that the employee saw the doctor on a Tuesday, but they state that the employee was "totally incapacitated" on the Monday through to Wednesday and can return to work on the Thursday. I actually took the note to the doctors office and asked the receptionist about them, showing her the notes. She immediately recognized the person by her first name and the quickly confirmed that these were legitimate sick notes, without looking anything up. When I asked about the signatures, she said that they just print off the signature like that (ie. no original signature). To me this is ridiculous, as anyone could write their own notes and just copy and paste an old electronic signature to the document. I am definitely suspicious that the receptionist and the employee are friends and came up with this scheme. What I am hoping to find out is whether there are standards for these types of official documents provided by doctors?
