You need to find his next of kin. You aren't his next of kin anymore because of the separation although you might to try and argue that you are given there is no signed separation agreement.
If he has adult children then they are the next of kin, of not, it would be his parents or siblings.
The Will isn't relevant and you cannot be a POA as he has to appoint someone to do that.
You could apply to be appointed as his legal guardian but that can't happen today and generally the hospital will take instructions from who is closest to him and so Court is really only used if there is a conflict.
Does he have a living Will? Do you know what his wishes are?
Generally, there shouldn't be much if anything for you to sign unless there is going to be a radical change in care. For example, if it's decided that he no more medical intervention be conducted and he is only to be made comfortable, then in cases such as this someone will be required to sign. But, if these are standard tests generally they don't require a signature so I am not sure what is going on exactly. Do you?
