One thing I want to start off by telling you is that your should insist that the bank card be switched to Bertram Starke. It is better to have all your banking information the same.
Because you passport has Bertram V. Starke as does all the financial information, your real only problem is the driver's licence. It's a piece of photo id that you will find you need frequently and so it's really important to get this changed.
I would suggest that you take your passport, SIN card, credit cards etc and insist that it be changed. Don't use your birth certificate or citizenship card.
Take it back to where you first had them change it and if that doesn't work you will need to go above the person's head and possible to the Ministry of Transport.
If you run in to further problems just post again. If I am missing something just reply back now and let me know.

I meant just for getting the driver's licence issue clarified. If they see that all your important id has the same name and not your longer birth name it won't be such a big deal.
