Whether or not he's on the birth certificate doesn't change whether or not he's the biological father, so that fact isn't evidence one way or the other.

He doesn't want to go on about how she moved away without his permission or how she's been denying him access, because he's taking the position that he's not the father apparently. The response to the Application should simply state that he wants a paternity test and he's willing to pay for it, and that if he's shown to be the father then he will make his position on the other issues clear with an amended response/answer.

Yes, you can certainly correct any mistakes on the basic data in your response. There should be a spot where you say that you agree with the basic facts she's laid out about their birthdates and such, if not then there should be a spot to say what's incorrect and what it should be.

If it's just the current address which is wrong, then use the proper address on his documents.

This paternity thing is dicey. Is he grasping at straws, here? Does he have any actual reason to believe that the child isn't or might not be his?

Then he'd better be explicit and convincing in his materials about why he thinks the child isn't or might not be his. He absolutely needs to bring that up in his paperwork. Just like the birth certificate issue, the amount of retroactive child support which he might have to pay isn't evidence as to whether he's the father of course.

I can appreciate that he "wants to be sure", but in my experience 95% of the named fathers who ask for a paternity test have no good reason for wanting one, they are just hoping that the child isn't theirs. So your partner should be clear about what his reasons are for wanting the test. Also, he should volunteer to pay for it so that the court knows he's serious. If it turns out he's not the father then he can seek an order that his ex pay him back, but frankly that's not a big deal because that would be the best money he ever spent. Most tests upon which the court will rely cost less than $500 these days.
