because I keep bringing to people who don't know what the code means. it usually happens when i'm going down a hills or steep decents, and the car becomes sluggish, So I would like to find out what exactly is the problem. I have changed transmission oil, filter, battery, crankshaft sensor and the problem is still there.

The problem is that your third gear is slipping. The expected input vs output speeds of the the transmission are not matching what the compute expects
You can check for any sign of oil in the trans electrical connector as this can cause weird errors but most likely the transmission will need to be rebuilt
