In 2013 whilst I was a registered nurse, I made the terrible/stupid mistake of fraudulently signing a medical practitioners signature for a blood pathology form. The company I was with reported it to Ahpra and an investigation was to be conducted. At that time I decided not to re-new my Nursing registration and the matter was never dealt with.
