9:30 am Dental appointment.
Re :- Susan Thomson accident.On the date and time indicated Susan Thomson had just finished having 2 fillings done and was sitting in the dental chair waiting for the Dentist to organise her next appointment when the Dental Assistant asked her to stand up and take a seat off to one side so she could begin to clean up, the seat concerned was on castors and was little more that a low stool with a very low back pad, no arm rests and on a hard vinyl tiled floor. The whole area was no more than about 4 metres by 4 metres with very little room to manoeuvre with the dental chair an x-ray machine, the Assistants stool, the Dentists stool, a computer desk and a four legged chair by one of the two access doorways.Susan stood up as requested by the Dental Assistant who positioned the stool behind her, the Assistant then turned around and proceeded to clean up the dental chair, Susan thinking the stool behind her was in fact a standard 4 legged chair and being held by the Assistant began to sit, as she did the stool rolled away from her and was by this time past the point of no return, with no seat behind her she fell hard onto the legs and castors of the stool.Susan suffers from M.S. and Ostioporosis which does cause her restrictive movement, poor balance and tactile sensation a fact that is very well known and on record with the dental centre.
Within seconds the centres Health Weathfare and Safety co-ordinator was by her side and on the floor placing her in the recovery position and directing someone to call for an ambulance, Susan was at this stage aside from any pain or injuries in shock and was not able to accurately indicate any major injuries or the true level of pain she was in.
Within 20 minutes to half an hour the ambulance and Paramedics were on site and assessing the situation and taking her vital statistics. After probably another 20 to 30 minutes helped to her feet and placed on the four legged chair by the entrance and trying to assess her condition and make a call as to whether she should go to the hospital for further treatment.Susan not wishing to take up the Paramedics or ambulance’s valuable time and not thinking she was seriously injured decided that she just wanted to go home and lay down in her own bed. However that night and a couple of Endone (5 mg). Later she started to feel uncomfortable and in severe pain, and said that maybe she had made the wrong call but would leave it to the following morning before making the call, by this time she was in no shape to get into our car for the drive to hospital in a sitting position, so reluctantly after an uncomfortable night we called for an ambulance and took her for further examination to the hospital.At the Gold Coast Uni. Hospital the first action was to get her pain under control and followed this up by having X-Rays taken of her lower back, primerally her coccyx bone. The X-Ray showed no magor damage, and was told that a CT Scan may be needed should the issue not clear up and that there was no treatmennt for the coccyx bone problem in any case.A couple of weeks latter on Susan's next visit I spoke to thier Health, Weathfare and Safety co-ordinator and asked for a copy of the Incident Report, she told me that action had been taken but was yet to be written up and that I would be coppied with the information as soon as it had been, not hearing from them after a couplt of weeks I contacted the Clinic to find out if I could recieve a copy of the Incident Report and any follow up action that may have been undertaken as a result of this issue and was told that there was no Incident Report but that there had been a memo sent to all Departments and all staff outlining new polacy and proceedures regarding the seats in question, I asked if it was posible for me to get a copy of this memo but 6 months later I have recieved nothing.
