My sympathy to you.
In respect of the matter the coroner will need to find fault with the doctor. My view is that a form is a simple standard procedure that a patient must write in and outline what is going on.
At 53, heart issues and the like should have been questions raised regardless of the form or not.
In short, they should have made the searches and check ups for mine.
In respect of the matter the coroner will need to look into it and see if it will recommend any further action be taken.
Such action can be charging the doctor criminally, as well as outlining an insurance claim also.
