My 2008 turbo diesel Toyota ace has been having problems

Hi, my 2008 turbo diesel Toyota hi ace has been having problems for some time now( well over 12 months). It started off intermittently, but now is constant every day.
When accelerating, it feels as though there is something blocking the fuel or air, and the engine feels like it is choking, therefore losing power, and feeling like it wants to cut out. From going up the gears, to accelerating at around 100km, it is running very poorly.
I have sprayed engine conditioner into the throttle body, and changed the fuel filter numerous times, along with using a injector cleaner frequently. I have no ideas what it could be

Have you done a basic tune up like air filter , air inlet checked for blockage , seen plastic bags get sucked in and block the ait to the airflter box.

Is the engine smoking when you try to accelerate ,if yes what colour ..

Any warning lights on when the engine is running like the check engine light ..

Can you get hooked up to a diagnostic scan tool and check for malfunction codes .

You now may have a problem with the MAF sensor .
