If it goes into reverse hard then the transmission may be in limp in. The trans will go into limp in if the trans controller sees an issue that can cause potential damage or it thinks it doesn't have complete control. There are many possible causes for limp in including solenoids, wiring, internal transmission, speed sensors, and the trans controller itself.

I don't have any reference for any of the codes you listed for the vehicle listed. What module did you find these in?
