Galaxy S4 safe mode won't turn off. What to do?

My boyfriends galaxy phone went into safe mode. Why? and how do you get rid of it??

This can be caused by a number of reasons
First, remove the battery from the phone and wait 5 minutes
Then put the battery back in, turn on the phone and tell me if it is still in Safe Mode please.

but what are the reasons for it coming into the phone?

Either it was accidentally put into Safe Mode or a hardware component crashed.

Phone model: SGH-M919N. I just restarted his phone and its not there anymore.... how do we prevent this from happening again. Also, he has 2 "message" icons, how did that happen.
I turned safe mode off this morning .... but not sure how this happened to begin with...

When its starting if you tap the menu key a few times it will put it into safe mode.
Or if it had a hardware failure.
These are the 2 ways it can get into this mode.

What do you mean by hardware failure? Two messaging icons means two different programs? But its the same icon... how could that be? Is there a way to permanently disable Safe Mode?

something in the hardware had a failure, crashed, it can happen.
Most messaging programs use the same icon.
No, you cannot disable safe mode.

There was a spy ware that was placed on his phone.... would this be causing it? it was working fine for many months, but now all of a sudden, the phone keeps going into safe mode. Does mspy cause this to happen on his phone?

It may be the case if the phone was hacked and corrupted the firmware and in this situation, you would need to reinstall the firmware.
This would remove all data and return the phone to new, but you cannot avoid this.
You would do the following
From your home screen press the apps icon in the lower right corner.
Find and press the settings icon.
In the upper end of the screen press the accounts tab.
in the bottom of the next screen press the Backup and reset button.
Press the factory data reset.
Read the info on your cell phone screen and hit Reset device.
Then set it up again
Myspy would not cause this. And if the problem continued its a hardware fault and would require the phone to be replaced.
