Likely your EGR cooler is leaking internally, and the coolant is being routed through the combustion chamber.

Does it do permanent engine damage? Does it require a change out of the EGR cooler?

I would inspect the EGR cooler, if it has coolant going through it the exhaust passages should be noticeably cleaner than they should be. You would need to replace it.
it can cause damage, as you can either run out of coolant and overheat, or you could be getting too much into the combustion chamber and hydrolock the engine, bend a piston rod or valves.
