Hello.

With intermittent periods of holding up a rear leg in a small dog like this, the most common cause is a luxating patella. This is where the knee cap (patella) slips in and out of the groove it normally sits in. This is very common in small/ toy breed dogs.

Most of these cases are managed just with exercise restriction or pain meds when they have an episode of lameness. In more severe cases, we do surgery to deepen the groove that the knee cap sits in or tighten the ligament holding it in place.

I would have a vet evaluate his knee and see if he will do better with medical or surgical care. My guess is that he'll be medical as his signs sound pretty mild and intermittent.

I hope this helps, let me know if you have any other questions.
