I have been asked to wtite a letter to a crown court Judge

I have been asked to wtite a letter to a crown court Judge in relation to my daughter who is to atend court on wednesday. I need to know how to begin the letter, as i am hand writing it, Wher do i write my name and address, date etc, i really dont know. Could anybody help me please
