Does this happen with all sources/inputs?
Tv would not be the only input, as you mentioned hdmi cable.Tv mode would be with an antenna only.
I am asking what sources/inputs are used (cable/satellite, dvd, game, media/apps, etc). I need you to specifically test multiple components and le tme know if the issue happens on all, or at least on more than one.
Can you please test something else, other than the dish box.
Please also unplug power to the dish box 60+ seconds to reboot it (not just tv)
Have you tested something else, other than the hopper?
Headphones work connected to what?I am asking you to complete a very specific test, using something other than the hopper with the tv.
I need that detail to be able to properly diagnose the problem if you wish to proceed.
