HL-S5086W
I have three lights flashing, replaced bulb (that was replaced three months ago). Turn the tv on, the lamp indicator light flashes for approximately 30 seconds, then the standby temp light comes on for approximately 5 seconds, the lamp indicator light comes on flashing for another 30 seconds, again goes to the standby light which stays on for approximately 5 seconds, then all three lights start flashing. During this process the screen stays black.

Let's see if we can narrow it down.

Turn the TV off, then put your face down by the lamp door... then turn the TV back on. You should hear the color wheel spin up (uh-wheeeee) like a high speed fan, then you should hear the ballast (click.... click.... click like TV is trying to turn on). Listen closely, these can be faint. Do you hear both?

I ordered it from DLP Lamps.com and it came from NESS Electronics Inc. I ordered it on June 25 and it arrived yesterday, June 29.

You may have gotten a bad lamp.

When the TV is first turned on, you heard the color wheel, right? Was it abnormally loud? Screaming at you?

The TV should "try" to light the lamp 3 times before giving up and flashing those 3 lights. Is that what's happening?

Everything you've described is 100% indicative of a bad lamp. Get your money back... it must've come with a warranty. Order one from this link. I have no affiliation with that company, but I know for a fact they sell genuine replacement parts and I have referred thousands of customers there over the years with very few complaints.
