Hello and welcome to JustAnswer!

If all of these issues started at the same time it's from electrical noise due to a failing alternator. A failing alternator will induce a/c current into the transmission output speed sensor circuit. This will cause the speedometer fluctuation and since the TCM sees output speed without input speed it will set a gear ratio error fault code and lock you in second gear. Disconnect the alternator field connector when this happens and if the speedometer settles down you'll need to replace the alternator.
